IMPORTANT INFORMATION FOR RETURNING STUDENT-ATHLETES

IMPORTANT INSURANCE/MEDICAL INFORMATION NEEDED PRIOR TO FALL 2009

The MSU Athletic department needs important health insurance information from you before you will be able to practice this coming fall. Please read the instructions below carefully and make sure you complete all steps. With the new process, you will have access to important forms that must be completed and returned to us prior to your arrival in the fall.

STEP 1: Go to the MSU athletic department student-athlete webpage found at: http://www.msubobcats.com/studentathletes.php

STEP 2: Scroll down until you see this heading: Athletic Training Room Forms **students should complete prior to 2009-2010**

STEP 3: Under returning student-athletes, click on Returning Student-Athletes Click Here First For Instructions. This link will take you to a page that will walk you through the process.
